Modification Form Needed for Windows!!

Alteration-Modification (Alt-Mod) forms are required for all window replacement.
Owners may be subject to expense of revisions or violations, if windows are installed without submission of the Alt-Mod form and the approval of the Board of Directors.
Bi-Laws Article VI Restrictions, Section 3. Alterations and Modifications: No Co-owner shall make alterations in exterior appearance or make  structural  modifications to any  Unit  (including  Residences  within  Detached  Units or make changes in the appearance or use of any of  the Common  Elements; Limited or General without the expressed written approval of the Board of Directors, including but not limited to, exterior painting, replacement of windows  or  doors,  or  the  erection  of  lights,  awnings,  shutters,  doors newspaper  holders,  mailboxes,  spas,  hot  tubs,  decks, patios,  structures,  fences,  walls  or  other exterior attachments  or  modification.
Association Rule 15.  Alteration/Modification Process
A.   Thirty (30) days prior to work commencing:
1)   Access www.herriman.net to obtain a copy of the Alteration/Modification Request Form.
2)   Complete an Alteration/Modification Request Form.
3)   Attached plans and specifications, showing the location, nature, shape heights, form of changes to the Alterations/Modification Request Form.
4)   All Alteration/Modification Request Forms and plans can be faxed, mailed, or emailed to our
designated management company:

B.   Our management agent will forward the Alteration/Modification Request Form and associated
paperwork to the Board of Directors within two (2) business days of receiving the request.
